MAT/HIV Program Case Manager

Lutheran Social Services of Central Ohio is currently seeking a MAT/HIV Program Case Manager for The Health Center at Faith Mission in Columbus, Ohio. Faith Mission is one of the largest homeless shelters in Columbus, Ohio, providing meals and shelter to men, women and Veterans with dignity and compassion.

REQUIREMENTS for a MAT/HIV Program Case Manager with Lutheran Social Services:

  • Bachelor's degree in human service field required.
  • LSW preferred.
  • Minimum one year working in a social service setting required.
  • Minimum one year in health care setting preferred.
  • Minimum two years' experience as a case manager or behavioral health professional preferred.
  • Experience working with the following populations preferred: homelessness, behavioral health, substance use, individuals living with HIV/AIDS.
  • Experience utilizing (minimally) the following techniques, interventions and EBP's: Motivational Interviewing skills, Stages of Change, Harm Reduction.

RESPONSIBILITIES for a MAT/HIV Program Case Manager with Lutheran Social Services:

  • Work with patients referred by Health Center providers to improve health and wellness
  • Screen patients for substance use disorders.
  • Utilize harm reduction, social, environmental, and motivational, strategies to support clients, and provide education as needed, in collaboration with licensed clinicians, counselors, and support staff.
  • Encourage and promote an environment that is strength-based to assist clients in meeting their individual goals.
  • Coach clients individually on problems with transition related to housing, supportive independent living skills, sobriety, and accessing needed resources in the community.
  • Develop treatment plans and collaborate with interdisciplinary team to track progress and goals.
  • Regularly update treatment plans, and facilitate services to assist with clients meeting treatment plan goals and reductions of barriers to treatment.
  • Assist in program quality assurance activities and gathering of program data.
  • Assist patients in obtaining social supports and benefits, such as health insurance, addressing food insecurity, etc.
  • Assist with Ryan White applications, as needed.
  • Coordinate and follow-up on referrals to specialty or outsides service providers.
  • Participate in weekly team huddles to conduct case review of patients enrolled in behavioral health program.
  • Provide access to emergency financial assistance (e.g. food vouchers, utility payment assistance, and transportation vouchers) as needed to promote and maintain positive health outcomes.
  • Provide quality and detailed documentation of contacts with patients, in a timely manner in the electronic health record and on paper as needed.
  • Participate in quality improvement and case review activities, working with colleagues to improve quality of care and client outcomes.
  • Contributes to an atmosphere of safety, sanctuary, dignity, and support through adherence to the Faith Mission Code of Ethics and Confidentiality Agreements.
  • Facilitate relationship with partnering organizations and develop referral partnerships to enhance and support growth of the MAT and HIV programs.
  • Participate in marketing and outreach activities as needed.
  • Performs other duties as required.
