HIV Prevention Case Manager

Lutheran Social Services of Central Ohio is currently seeking a HIV Prevention Case Manager for The Health Center at Faith Mission in Columbus, Ohio. Faith Mission is one of the largest homeless shelters in Columbus, Ohio, providing meals and shelter to men, women and Veterans with dignity and compassion.

REQUIREMENTS for a HIV Prevention Case Manager with Lutheran Social Services:

  • Bachelor's degree in human service field required.
  • LSW preferred.
  • Minimum one year working in a social service setting required.
  • Minimum one year in health care setting preferred.
  • Minimum two years' experience as a case manager or behavioral health professional preferred.
  • Experience working with the following populations preferred: homelessness, behavioral health, substance use, individuals living with HIV/AIDS.
  • Experience utilizing (minimally) the following techniques, interventions and EBP's: Motivational Interviewing skills, Stages of Change, Harm Reduction.

RESPONSIBILITIES for a HIV Prevention Case Manager with Lutheran Social Services:

  • Work with patients referred by Health Center providers to improve health and wellness.
  • Screen patients for HIV by utilizing rapid testing.
  • Provide sexual health screening to all new patients.
  • Provide education and prevention information to patients at high risk for contracting HIV and sexually transmitted illnesses (STIs).
  • Assist patients interested and eligible for PrEP in obtaining the medication and collaborate with providers to assist in eliminating barriers to PrEP.
  • Utilize harm reduction, social, environmental, and motivational, strategies to support clients, in collaboration with licensed clinicians, other counselors, case managers, and support staff.
  • Collaborate with BH team and health center staff regarding treatment and care.
  • Encourage and promote an environment that is strength based to assist clients in meeting their individual goals.
  • Coach clients individually on problems with transition related to housing, supportive independent living skills, sobriety, and accessing needed resources in the community.
  • Conduct outreach to identified clients working with the team to engage in services and accept treatment from the program staff assist with enrollment process as needed.
  • Assist in program quality assurance activities and gathering of program data.
  • Verify enrollment in medical coverage, and support enrollment of the uninsured in Basic Health Care; including enrollment in patient assistance and other programs to assist with medication costs.
  • Schedules patients for follow-up, including medical, dental, and vision appointments.
  • Conducting counseling, supportive services, and monitor based on individual need.
  • Coordinating information and care requirements with other care providers.
  • Assist in resolving issues that could affect smooth care progression.
  • Coordinate and follow-up on referrals to specialty or outsides service providers.
  • Provide access to emergency financial assistance (e.g. food vouchers, utility payment assistance, and transportation vouchers) as needed to promote and maintain positive health outcomes.
  • Provide quality and detailed documentation of contacts with patients, in a timely manner in the electronic health record and on paper as needed.
  • Participates in quality improvement and case review activities, working with colleagues to improve quality of care and client outcomes.
  • Contributes to an atmosphere of safety, sanctuary, dignity, and support through adherence to the Faith Mission Code of Ethics and Confidentiality Agreements.
  • Assists with crisis intervention and de-escalation as needed by providing leadership, and by modeling professional and calm problem-solving behavior for clients, colleagues, and the community.
  • Refer and link clients to appropriate services within the system of care that promote positive health outcomes, treatment adherence, and greater self-sufficiency. Monitor the client's follow-through with these services.
  • Facilitate relationship with partnering organizations and develop referral partnerships to enhance and support growth of the program.
  • Participate in marketing and outreach activities to promote the HIV Prevention program as needed.
  • Provide input and assist in policy and program development.
  • Performs other duties as required.
